2024 Community Health Assessment

Do you live, work or play in Monroe County? Do you want to build a healthier tomorrow?

You are invited to any of the upcoming Community Conversations (or focus groups) to discuss the everyday events that contribute to your health and well-being or that of your family friends, and others in Monroe County. 

Information gathered will be incorporated into the next Community Health Assessment which will be used to create an action oriented Community Health Improvement Plan to be implemented over the next 3 years. 

As of March 8th, 2024, the following locations for community conversations/ focus groups have been scheduled:

No registration is necessary. Please note that these times/days are subject to change. Please contact khewett@co.monroe.in.us with any questions regarding the focus groups.

Community Health Improvement Plan (CHIP), August 2022 + 

According to CDC, 'a community health improvement plan (or CHIP) is a long term, systematic effort to address public health problems based on the results of community health assessment activities and the community health improvement process.' 

Monroe County Health Department along with community partners and community members has initiated the CHIP planning process as of August 2022!

Think Tanks, April 2022

The Monroe County community was invited to two Think Tank events held in April 2022 to identify three areas of focus for the CHIP workgroups based on information from the Monroe County Community Health Assessment (CHA), targeted focus groups, intercept interviews and secondary data collection.
