Futures Family Planning Clinic

The Futures Family Planning Clinic provides family planning health services for adolescents and adults. Birth control counseling and supplies available on the sliding fee schedule include pills, shots, patch, diaphragm, vaginal ring, subdermal implant (Nexplanon), and IUD (intrauterine device).  Condoms provided at no charge. 

*Insurance and Medicaid are accepted.*

Appointments are strongly preferred. Please call (812) 349-7343 to schedule. Walk-ins are accepted ONLY if an appointment time is available.

NOTE:  A Spanish speaking medical interpreter is available by appointment only.

• Exams including pap, breast exam, pelvic exam
• Pregnancy counseling, testing and referral
• Sexually transmitted disease testing and treatment, including HIV testing, gonorrhea and chlamydia
• Emergency contraception (Plan B)
• Health education
• Birth control counseling, services, and supplies 

Health Insurance Assistance

A Certified Health Insurance Navigator is available to answer questions, provide referrals, and assist with enrollment in Healthy Indiana Plan, Hoosier Healthwise, and the Federal Health Insurance Marketplace.
