IU Health Bloomington, Monroe County Health Department, City of Bloomington, CJAM and Health Net are partnering to host the 2021 Community Health Needs Assessment that will drive our work over the next three years.
Your input is needed because gathering insight from community members about the greatest health needs facing Monroe County is essential in this process.
This process defines the community health priorities for IU Health Bloomington Hospital, the Monroe County Health Department, the City of Bloomington, and the Community Health Improvement Plan (CHIP) teams for the next three years.
