• Uncontested Divorces:
  Sample forms are available at the Monroe County Public Library, and will need to be retyped before filing with the court.
  You must have copies of the following when filing:
  • Petition for Dissolution (1 original and 2 copies)
  • Decree of Dissolution (1 original and 2 copies)
  • If Child Support is to be received, the decree of dissolution must include the following statement:
    Payor of Support must pay an annual docket fee to the Clerk pursuant to IC 33-19-6-5
