Birth and Death Certificates Minimize

The Monroe County Health Department ONLY accepts CASH, CHECK or MONEY ORDER; NO credit or debit cards

You can ONLY apply In-Person or Through The Mail

Most transactions take 5 minutes In-Person. Affidavits can take up to 30 minutes.

We are open 8am to 4pm, Monday through Friday; except on holidays. We do not close for lunch.

Mail takes as long as the mail takes. We process requests same day received from the mail.

Birth Certificates ~ $15.00 per set; you will receive a wallet size and a half sheet.

Death Certificates ~ $15.00 per copy; corrections must be made by funeral home that provided services.

     *Please contact the funeral home you are working with directly for any delay in receiving death certificates.*

Paternity Affidavit ~ $52.00; to file in office with both parents present, identification required.

Paternity Affidavit Upon Marriage ~ $27.00; to file in office with both parents present, identification required and certified copy of marriage license.

Certified Copy of "filed" Paternity Affidavit ~ $10.00 per copy; obtained only by parents with proper identification.

Affidavit Requesting Amendment ~ $27.00; to file in office with 2 proofs of minor correction that need to be 10 years or older for birth certificates only.

Amendment by Notification (if Child is less than 2 years old) ~ $10.00

***Please call ahead when filing a Home Birth to receive instructions and to make an appointment. The fee to register a home birth is $25.00.

 

Certificates and Fees

The Vital Statistics Division provides birth and death information ONLY for Monroe County, Indiana from 1882 to Present.

Print and complete the Certificate Application required by Monroe County Health Department for the following documents:

             Birth Certificate Application

             Death Certificate Application

We ONLY accept cash, money orders and checks with driver's license in the office. NO credit or debit cards will be accepted as payment.

Identification

Due to the increased identity theft and fraudulent use of documents, we must be diligent in our endeavors to protect the records we are responsible for issuing.  Therefore, in order to obtain a Birth and/or Death Certificate, you will need specific documentation at the time of issuance.

If mailing, please enclose a photocopy of your Driver's License or State issued ID card or Passport.

Primary Documentation MUST be current and valid; one of the following is needed:
  • Valid Driver's License
  • Valid State issued ID card
  • Valid Work ID (Must be still employed)
  • Military ID with Signature
  • Valid School ID ( Must be attending)
  • Veterans ID card
  • Mexico Consular ID card
  • Corrections ID card
  • Probation ID card
  • Valid Passport
Secondary Documentation may be accepted in the event an individual has lost their Primary Documentation due to theft, fire, etc. two or more of the following is needed:
  • Hunting or Fishing License with your signature on it
  • Employee ID Card
  • Voter Registration Card
  • Medical Insurance Card
  • Car registration signed by person obtaining certificate
  • Previous year tax return with signature and Social Security Number
  • Social Security Card
  • Gun Permit with Signature
  • Club Membership with Photo and Signature
  • Police Report (if individual reports their identification was stolen)
  • Fire Report (if individual reports their identification was destroyed by fire)
  • Bank Card or Personal Check with current information
  • Probation documents/statement from Probation Officer on letterhead including person's name and date of birth
  • Letter from Social Security Administration or BMV that shows individual’s name, date of birth, parent’s names and signature
  • Expired Driver’s License
  • Arrest Fact Sheet with photo from Sheriff’s Department
If none of these documents can be obtained a court order stating “Monroe County Health Department must release the birth certificate to the individual named in order” has to be acquired.
 
Payment
For the Health Department to accept a personal check through the mail, we require the person signing the check to include a photocopy of their Driver's License or State Issued Identification Card.
 
Without a valid Driver's License or State Issued Identification Card, you must pay with a Money Order, Cash or Cashiers Check instead of a personal check.
 
You will need to mail the application with all the correct information, fees and identification requirements to the following address:

We will mail the certificate on the same day we receive all of the correct and complete information from you through the mail.

Monroe County Health Department does not accept credit cards or online orders; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service; VitalChek Network Inc. VitalChek can be reached through its website www.vitalchek.com. An additional fee is charged by VitalChek for using this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard®, or Visa®. 
 
 
 
 
You may order a certified copy of a birth or death certificate online using your credit card through our only approved Internet ordering service, VitalChek Network Inc.
  
Other internet vendors are not authorized by the Monroe County Health Department and may charge additional fees without providing state-approved services. Neither MonroeCounty nor VitalChek have control over the services and fees other Internet vendors offer and charge, nor can we guarantee the confidentiality of any vital record data provided to vendors other than VitalChek.
