Birth and Death Certificates Minimize

The Monroe County Health Department accepts CASH, CHECK, MONEY ORDER and MOST DEBIT AND CREDIT CARDS. 

You may apply in person, through the mail, or through VitalChek.  (See bottom of page for information regarding VitalChek)

Most transactions take approximately 5 minutes in person. Affidavits and home birth registration can take up to 30 minutes.

We are open 8 a.m. to 4 p.m., Monday through Friday; except on holidays. We do not close for lunch.

Most requests received through the mail are processed on the day received.

Birth Certificates ~ $15.00 per set; you will receive a wallet size and a half sheet.

Death Certificates ~ $15.00 per copy; corrections must be made by funeral home that provided services.

     *Please contact the funeral home you are working with directly for any delay in receiving death certificates.*

Paternity Affidavit ~ $52.00; to file in office with both parents present, identification required.

Paternity Affidavit Upon Marriage ~ $27.00; both parents present, identification required, certified copy of marriage license, mother's divorce decree, if applicable.

Certified Copy of "filed" Paternity Affidavit ~ $10.00 per copy; obtained only by parents with proper identification.

Affidavit Requesting Amendment ~ $27.00; two documents at least 10 years old showing correct information.  For children 3 years old through 17, call office for information.

Amendment by Notification (if Child is less than 2 years old) ~ $10.00

***Please call ahead when filing a Home Birth to receive instructions and to make an appointment. The fee to register a home birth is $25.00.

 

Certificates and Fees

The Vital Statistics Division provides birth and death information ONLY for Monroe County, Indiana from 1882 to Present.

Print and complete the Certificate Application required by Monroe County Health Department for the following documents:

             Birth Certificate Application

             Death Certificate Application

We accept cash, money order, checks with check-writer's valid ID, and most major credit cards.

Identification

Due to the increased identity theft and fraudulent use of documents, we must be diligent in our endeavors to protect the records we are responsible for issuing.  Therefore, in order to obtain a Birth and/or Death Certificate, you will need specific documentation at the time of issuance.

If mailing, please enclose a photocopy of your Driver's License or State issued ID card or Passport.

Primary Documentation MUST be current and valid; one of the following is needed:
  • Valid Driver's License
  • Valid State issued ID card
  • Valid Work ID (current)
  • Military ID with Signature
  • Valid School ID ( current)
  • Veterans ID card
  • Mexico Consular ID card
  • Department of Corrections ID card
  • Probation ID card
  • Valid Passport
Two Secondary Documentation may be accepted in the event an individual does not have primary documentation.
  • Hunting or Fishing License with signature
  • Employee ID Card
  • Voter Registration Card
  • Medical Insurance Card
  • Car registration signed by person obtaining certificate
  • Previous year tax return with signature and Social Security Number
  • Social Security Card with signature
  • Gun Permit with Signature
  • Club Membership with Photo and Signature
  • Police Report (if individual reports their identification was stolen)
  • Fire Report (if individual reports their identification was destroyed by fire)
  • Bank Card or Personal Check with current information
  • Probation documents/statement from Probation Officer on letterhead including person's name and date of birth
  • Letter from Social Security Administration or BMV that shows individual’s name, date of birth, parent’s names and signature
  • Expired Driver’s License
  • Arrest Fact Sheet with photo from Sheriff’s Department
 
Payment
If paying by check, must provide check-writer's valid driver's license.
 
Without a valid Driver's License or State Issued Identification Card, you must pay with a Money Order, Cash or Cashiers Check.

Other internet vendors are not authorized by the Monroe County Health Department and may charge additional fees without providing state-approved services. Neither Monroe County nor VitalChek have control over the services and fees other internet vendors offer and charge, nor can we guarantee the confidentiality of any vital record data provided to vendors other than VitalChek.
