Vital Statistics

 

General Information
The Vital Records Staff is involved in the preparation, processing, and issuance of licenses, permits, certificates, and Certification Cards for the Environmental, Food Protection Staff, and the Registered Septic Installers.

Certificates and Fees

The Vital Statistics Department provides the following for events that occurred in Monroe County, Indiana from 1882~Present

Birth Certificates ($15.00 per certified copy) 

Death Certificates ($11.00 per certified copy)

This department also offers Birth & Death Genealogical Research Records at the cost of $15.00 for up to three searches.  All persons obtaining any of these documents must have a form of valid ID in order for the Vital Records Staff to issue the certificates. Due to the increased identity theft and fraudulent use of legal documents, we must be diligent in our endeavors to protect the records we are responsible for issuing.  Therefore, in order to obtain  Birth and or Death Certificates there are specific documents that must be presented at the time of issuance and they are as follows:

~Primary  Documentation, which must be current and valid~

  • Driver's License
  • Military ID
  • State issued ID card
  • School ID w/Photo
  • Department of Correction ID, issued within the past 6 months.
  • High School Yearbook w/your photo in it
  • Court Order must order local health department to release record to person names on the record.

~Secondary Documentation, in the event an individual has lost their primary ID due to theft, fire, etc we will accept two or more of the following:

  • Car registration signed by person obtaining certificate*
  • Voters Registration Card*
  • Medical Insurance Card*
  • Voters Registration Card
  • Hunting, Fishing, or Gun Permit with your signature on it.*
  • Employee ID Card
  • Welfare, Food Stamp or WIC ID card(s)
  • Previous years tax return with signature & Social Security Number
  • Social Security Card
  • Gun Permit with Signature
  • Club Membership with Photo & Signature
  • Police Report (if individual reports their ID stolen)
  • Fire Report (if individual reports their ID destroyed by fire)
  • Copies of signed employment application
  • Bank Card or Personnel Check with current information
  • Previous year's tax return with signature & social security number.
  • Probation documents/statement from Probation Officer on letterhead, including person's name & date of birth.
  • Letter from Social Security Administration or BMV that shows individuals name, date of birth, parents & signature.
  • Signed Lease or Loan Agreements.
  • Expired Drivers License

In the instance that you would not have a valid state issued photo ID, two forms of the above IDs will be required to obtain a Birth, Death Certificate or Genealogical Record.

Anytime a check is written it is required that a copy of the check writers Drivers License/State issued ID Card must be included with the application. Those without a valid Driver's License or State ID must provide a Money Order, Cash or Cashiers Check in lieu of a Personal Check.
